The Fall 2026 Dual Enrollment materials request survey opens on

Please use the form below to request both print materials and course access codes/eBooks. Once your request has been processed, the appropriate resources will be assigned.
You will be notified at your S# email when your materials are ready for pickup; all print materials and access codes/eBooks must be collected from our office. We are unable to email access codes.
After submitting the form, you will see a confirmation message indicating that your request was successfully received. Please note that we are unable to verify submissions by phone.
After ordering your materials and receiving an email indicating they are ready, you will be required to have a school ID or state ID to pick up the materials. Parents/Guardians and emergency contacts listed in the students HAC account are allowed to pick up materials/resources with proper ID.
**We are next to Sebastian Middle School – please use the correct gate to access our buildings. We are not located inside the school.
