Family Empowerment Scholarship

All households within the district receiving free or reduced priced meals under the National School Lunch Act are eligible  to participate in the Family Empowerment Scholarship for Educational Options (FES EO) Program.

**Applicants who have limited English proficiency**

This link features foreign language translations of the Prototype Application for Free and Reduced Price School Meals. They are provided by USDA as a template to assist state and local agencies in serving households where English is not spoken as a primary language. Households may also download these resources directly to be filled out and submitted to their local school district. Please note that the PDF forms have 2016-2017 SY listed, it’s the latest version from USDA. For any questions, please call the Food and Nutrition department at 904-547-8931.

Important Notice

Students that are enrolled at the following CEP (Community Eligibility Provision) schools do not need to complete an application for Free or Reduced Meals:

  • Crookshank Elementary School
  • Gamble Rogers Middle School
  • Hartley Elementary School
  • Ketterlinus Elementary School
  • Murray Middle School
  • Osceola Elementary School
  • Otis Mason Elementary School
  • Sebastian Middle School
  • South Woods Elementary School
  • St. Johns Technical High School
  • The Evelyn Hamblen Center (Formerly Transitions)
  • Webster Elementary School

All students at these locations eat at no cost.

 
 

Please Note the following change to the Free and Reduced Meal Application: If your household receives SNAP, FDPIR or State TANF cash assistance provide the Case number for the person who receives benefits.
