*Must submit termination form if currently a Home Ed student transferring to PEP or ending Home Ed program through transfer/affidavit. If you are not a previously registered Home Ed student and at a brick-and-mortar school in the district, withdraw from your zoned school directly for PEP.
Home Education Annual Evaluation
Only for Home Ed students, not PEP. PEP students should contact their Scholarship Funding Organization (SFO) for annual requirements.
*Evaluation is due every year to district Home Education Office on the anniversary date of the student’s enrollment in Home Education.

Transcript Templates
In a Home Education program, the parent or legal guardian serves as the custodian of the student’s educational records. This means you will create and maintain a transcript for each school year which lists the courses your child has completed as well as the final grades earned.
St. Johns County School District (SJCSD) will maintain transcripts only for any courses your child takes through cross-enrollment at their zoned school or through St. Johns Virtual School. If your child completes courses through Florida Virtual School, you can log in to your FLVS account at any time to print an unofficial transcript directly from their system.
If your child returns to full-time enrollment at a brick-and-mortar school, you will need to provide a transcript that you have created, that includes the courses completed, final grades awarded, and the grade level to which your child was promoted.
To assist you, we have provided sample transcript templates on this site, but you are not required to use any specific format. You may also find additional template options through various Home Education websites and resources.

Bright Futures Scholarship Form
PEP & Home Ed students must submit reflection form and service hours and/or paid work hours paystubs/letterhead to Home Ed email only during senior year after applying for Bright Futures Scholarship which opens Oct. 1 annually
