USDA provides them as a template to assist state and local agencies in serving households where English is not spoken as a primary language. Households may also download these resources directly to be filled out and submitted to their local school district.  The PDF forms have 2016-2017 SY listed, please note that this is the latest version available from USDA. Students that are enrolled at the following CEP (Community Eligibility Provision) schools do not need to complete an application for Free or Reduced Meals:

Please Note the following change to the Free and Reduced Meal Application:

If your household receives SNAP, FDPIR, or State TANF cash assistance, please provide the Case Number for the person who receives benefits.

Family Empowerment Scholarship

All households within the district receiving free or reduced-priced meals under the National School Lunch Act are eligible to participate in the Family Empowerment Scholarship for Educational Options (FES EO) Program.
