You will create a secure username\/password to enter the system.<\/p><p>Enter\u00a0this code when you make your appointment:<\/p><p style=\"text-align: center;\">Fieldprint Code:<strong> FPStJohnsVolunteer<\/strong><\/p><p>Note &#8211; Fieldprint Codes are case-sensitive.<\/p><p>On the\u00a0\u201cReason For Fingerprinting\u201d\u00a0screen, enter the\u00a0Fieldprint Code\u00a0provided above.<br \/>Complete all required fields as requested. The information you provide is required by the Florida Department of Law Enforcement (FDLE) and the Federal Bureau of Investigation (FBI) to conduct a state and national criminal history check.<\/p><p>Select a local collection facility to schedule and pay for your appointment (see below for payment information). You will receive instructions, directions, maps, and photos for your appointment online.<\/p><p>If you have any issues or questions, you may contact the customer service team at (800) 799-1067 or at\u00a0<a href=\"mailto:CustomerService@fieldprint.com\"><strong>CustomerService@fieldprint.com<\/strong><\/a>.
