the fingerprint process cannot continue without it.<\/span><\/li><li>Due to the sensitivity and confidentiality of the information requested, you must return these documents to the Volunteer Services Department\u00a0using one of the following options:<\/li><\/ul><ol><li style=\"list-style-type: none;\"><ol><li>By fax: <strong>904-547-3956<\/strong>, Attn: Volunteer Services Department<\/li><li>By mail: 40 Orange Street, St. Augustine, Florida 32084, Attn: Volunteer Services Department<\/li><li>In person: 40 Orange Street, St. Augustine, Florida 32084, Attn: Volunteer Services Department<\/li><\/ol><\/li><\/ol><p>The information requested will be entered into the Clearinghouse to determine if fingerprinting is required.\u00a0 After reviewing you will receive verification via email that: a) your fingerprints are acceptable in the Clearinghouse; or b) you need to have fingerprints done.\u00a0 If you need to have fingerprints done, proceed to Step 2.<\/p><p><strong><span style=\"text-transform: uppercase;\">Note \u2013 Do not proceed to step 2 until receiving verification that fingerprints are necessary.<\/span><\/strong><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-7e873b4 elementor-widget elementor-widget-heading\" data-id=\"7e873b4\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Step 2:<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-74abc5fe elementor-widget elementor-widget-text-editor\" data-id=\"74abc5fe\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Visit\u00a0<a href=\"https:\/\/www.fieldprintflorida.com\/\"><strong>www.fieldprintflorida.com<\/strong><\/a>. You will create a secure username\/password to enter the system.<\/p><p>Enter\u00a0this code when you make your appointment:<\/p><p style=\"text-align: center;\">Fieldprint Code:<strong> FPStJohnsVolunteer<\/strong><\/p><p>Note &#8211; Fieldprint Codes are case-sensitive.<\/p><p>On the\u00a0\u201cReason For Fingerprinting\u201d\u00a0screen, enter the\u00a0Fieldprint Code\u00a0provided above.<br \/>Complete all required fields as requested. The information you provide is required by the Florida Department of Law Enforcement (FDLE) and the Federal Bureau of Investigation (FBI) to conduct a state and national criminal history check.<\/p><p>Select a local collection facility to schedule and pay for your appointment (see below for payment information). You may also select the \u201cContact Us\u201d link on the website.<\/p><p><strong>Payment Information<br \/><\/strong>Payment can only be made by credit card.<br \/>The average cost for volunteer fingerprinting is $83.25.
