24pt\"><strong>Before continuing, please gather the REQUIRED items<\/strong><\/span><\/p>\n<p><span style=\"color: #ff0000\"><strong><span style=\"font-size: 14pt\">All drivers must upload a copy of your driver license (front only)<\/span><\/strong><\/span><\/p>\n<p><span style=\"color: #ff0000\"><strong><span style=\"font-size: 14pt\">If you wish to transport students in a personal-owned automobile, you must also upload insurance policy declaration pages, NOT a copy of your insurance card, with the following information:<\/span><\/strong><\/span><\/p>\n<ol>\n<li><span style=\"color: #ff0000\">Insured name(s)<\/span><\/li>\n<li><span style=\"color: #ff0000\">Insurance carrier<\/span><\/li>\n<li><span style=\"color: #ff0000\">Effective dates of coverage<\/span><\/li>\n<li><span style=\"color: #ff0000\">Policy number<\/span><\/li>\n<li><span style=\"color: #ff0000\">Minimum policy limits of 100\/300\/50 ($100,000 bodily injury per person\/$300,000 bodily injury per accident\/$50,000 property damage)<\/span><\/li>\n<\/ol>\n<p style=\"text-align: center\"><strong><span style=\"color: #ff0000\"><span style=\"font-size: 14pt\">Please note applications without a driver&#8217;s license uploaded will be deleted.<\/span><\/span><\/strong><\/p>\n<h1 style=\"text-align: left\"><span style=\"font-family: 'book antiqua', palatino, serif\">\u00a0<\/span><span style=\"font-family: georgia, palatino, serif;font-size: 18pt\">Requirements of <strong>All<\/strong> Volunteer Drivers<\/span><\/h1>\n<ul>\n<li style=\"text-align: left\"><span style=\"color: #000000;font-size: 12pt;font-family: georgia, palatino, serif\">Be an <strong><u>approved<\/u><\/strong> SJCSD volunteer, employee or adjunct coach\u00a0<\/span><\/li>\n<li><span style=\"font-size: 12pt;font-family: georgia, palatino, serif\">Possess a valid driver&#8217;s license<\/span><\/li>\n<li><span style=\"font-size: 12pt;font-family: georgia, palatino, serif\">Ensure all students wear seat belts<\/span><\/li>\n<li><span style=\"font-size: 12pt;font-family: georgia, palatino, serif\">The number of students transported will be limited by the specified capacity limits and corresponding seat belts provided<\/span><\/li>\n<li><span style=\"font-size: 12pt;font-family: georgia, palatino, serif\">Must have the names and phone numbers of all passengers<\/span><\/li>\n<li><span style=\"font-size: 12pt;font-family: georgia, palatino, serif\">Vehicle must be in good repair\u00a0\u00a0<\/span><\/li>\n<\/ul>\n<h2><span style=\"color: #339966\">Do:<\/span><\/h2>\n<ol>\n<li><span style=\"color: #339966\"><strong>Be Punctual<\/strong><\/span>\n<ul>\n<li><span style=\"color: #339966\">Arrive early to allow time for loading and introductions.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #339966\"><strong>Follow All Traffic Laws<\/strong><\/span>\n<ul>\n<li><span style=\"color: #339966\">Obey speed limits, stop signs, and use turn signals at all times.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #339966\"><strong>Ensure Vehicle Safety<\/strong><\/span>\n<ul>\n<li><span style=\"color: #339966\">Confirm the vehicle is roadworthy (tires, brakes, lights, seatbelts all working).<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #339966\"><strong>Use Seat Belts<\/strong><\/span>\n<ul>\n<li><span style=\"color: #339966\">Ensure every passenger, including yourself, is wearing a seatbelt before driving.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #339966\"><strong>Keep Emergency Contacts Handy<\/strong><\/span>\n<ul>\n<li><span style=\"color: #339966\">Have a list of emergency contacts for each student and the organization.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #339966\"><strong>Drive Defensively<\/strong><\/span>\n<ul>\n<li><span style=\"color: #339966\">Anticipate others\u2019 mistakes and maintain a safe following distance.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #339966\"><strong>Stay Focused and Alert<\/strong><\/span>\n<ul>\n<li><span style=\"color: #339966\">Eliminate distractions and take breaks if feeling tired.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #339966\"><strong>Communicate Clearly<\/strong><\/span>\n<ul>\n<li><span style=\"color: #339966\">Introduce yourself, state the route, and let students know expectations (e.g., no loud music, stay seated).<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #339966\"><strong>Respect Privacy<\/strong><\/span>\n<ul>\n<li><span style=\"color: #339966\">Keep any information about students confidential.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #339966\"><strong>Stay Calm and Polite<\/strong><\/span>\n<ul>\n<li><span style=\"color: #339966\">Model courteous behavior behind the wheel.<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<h2 style=\"text-align: left\"><span style=\"color: #ff0000\">Don\u2019t:<\/span><\/h2>\n<ol style=\"text-align: left\">\n<li><span style=\"color: #ff0000\"><strong>Don\u2019t Use a Phone While Driving<\/strong><\/span>\n<ul>\n<li><span style=\"color: #ff0000\">No texting, calling, or using apps without hands-free devices.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #ff0000\"><strong>Don\u2019t Speed or Drive Aggressively<\/strong><\/span>\n<ul>\n<li><span style=\"color: #ff0000\">Avoid tailgating, weaving, or sudden braking\/acceleration.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #ff0000\"><strong>Don\u2019t Transport Extra People<\/strong><\/span>\n<ul>\n<li><span style=\"color: #ff0000\">Only drive the students you\u2019re assigned and never exceed vehicle capacity.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #ff0000\"><strong>Don\u2019t Make Unscheduled Stops<\/strong><\/span>\n<ul>\n<li><span style=\"color: #ff0000\">No detours or stops unless pre-approved or in case of emergency.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #ff0000\"><strong>Don\u2019t Allow Unsafe Behavior<\/strong><\/span>\n<ul>\n<li><span style=\"color: #ff0000\">No standing, switching seats, or distracting the driver during the ride.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #ff0000\"><strong>Don\u2019t Leave Students Unattended<\/strong><\/span>\n<ul>\n<li><span style=\"color: #ff0000\">Always wait until students are safely handed off to a responsible adult or reach their destination.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #ff0000\"><strong>Don\u2019t Drive Under the Influence<\/strong><\/span>\n<ul>\n<li><span style=\"color: #ff0000\">Never drive under the influence of alcohol, drugs, or strong medications that impair ability.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #ff0000\"><strong>Don\u2019t Discuss Inappropriate Topics<\/strong><\/span>\n<ul>\n<li><span style=\"color: #ff0000\">Avoid political, religious, or personal conversations that could make students uncomfortable.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #ff0000\"><strong>Don\u2019t Assume Permission<\/strong><\/span>\n<ul>\n<li><span style=\"color: #ff0000\">Always follow the organization\u2019s policies on consent forms, pickup\/drop-off locations, and behavior guidelines.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #ff0000\"><strong>Don\u2019t Forget to Report Incidents<\/strong><\/span>\n<ul style=\"text-align: left\">\n<li><span style=\"color: #ff0000\">Ensure you know what to do if an incident occurs and report it as soon as possible.<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<p><span style=\"font-size: 14pt\"><strong>When Transporting Students in Vehicles Rented Through SJCSD<\/strong><\/span><\/p>\n<ul>\n<li>Covered through District Automobile Insurance<\/li>\n<li>Personal Automobile Insurance policy limits are not necessary and do not have to be provided<\/li>\n<\/ul>\n<p><span style=\"font-size: 14pt\"><strong>When Transporting Students in Personal-Owned Vehicles<\/strong><\/span><\/p>\n<ul>\n<li>Personal Automobile Insurance is primary<\/li>\n<li>Driver assumes personal financial liability and is responsible for loss and accidental damage to his\/her vehicle should an incident occur<\/li>\n<li>Provide Policy Declaration Page(s) (<strong><span style=\"color: #ff0000\">NOT an insurance card<\/span><\/strong>):<\/li>\n<\/ul>\n<p>Completion of a new request is not required each year.\u00a0 Once you\u2019re approved and added to the approved volunteer driver database you remain approved until y<span style=\"font-size: 16px\">our auto insurance expires <\/span>or y<span style=\"font-size: 16px\">our driver license expires.<\/span><\/p>\n<p>You will receive an email 30 days prior to the expiration of and again upon expiration of your auto insurance and your driver license.\u00a0 Once you respond to that email request with updated auto insurance declarations page(s) or an updated driver license, you will remain approved.<\/p>\n<p><strong><span style=\"color: #993300\">Please note:\u00a0 You are not approved to transport students in a vehicle rented through St. Johns County School District or your personal-owned vehicle until you receive an approval email from notifications@origamirisk.com.\u00a0 If you haven&#8217;t received an email within five (5) business days of application submittal, please check your SPAM folder.<\/span><\/strong><\/p>\n<p
