Immunization Requirements PDF Documents Summary of Recommendations for Childhood & Adolescent Immunization (PDF) Adapted from the recommendations of the Advisory Committee on Immunization Practices (ACIP)* by the Immunization Action Coalition, November 2006 Meningococcal Vaccine: What You Need to Know (PDF) Provided by the Department of Health & Human Services Centers for Disease Control and Prevention K through 4th grade DPT series, Hepatitis B series, 2 doses measles (preferably MMR), Polio series, Varicella (chicken pox) ALL students 5th grade and 6th grade DPT series, Hepatitis B series, 2 doses MMR, Polio series 7th grade entry Tetanus-Diptheria booster (Td)- (within past 5 years) (students entering after 7th grade- Td boosters recommended every 10 years) ALL students 5th grade through 12th grade- Hepatitis B series, 2 doses MMR, Td Booster *** Note: All students in Florida schools K through 12th grade are required to have the Hepatitis B series and two Measles (preferably MMR). *** Requirements are for grade level. Any student retained must comply with current grade.
