Only legal guardians may apply and enroll a child in the Head Start Program. Applications will only be processed if the required documents listed below are provided along with the completed application.
Certified Birth Certificate
Birth Certificates are available at the Office of Vital Statistics at the St. Johns County Health Department. Your child must be at least three years of age on or before September 1, 2011 to be eligible.
Two Proofs of Residence
Acceptable documents:
Driver's license
Utility bill
Lease agreement
Income
Proof of family income for 2010 or previous twelve months.

Benefits Award Letter
Pay Stubs
Employer Verification
Photo Identification
Photo Identification of the legal parent/guardian is required.
