Public Records Requests Would you like to request public records from our Community Relations Department? You may send a request to us online, by FAX, mail or in person. Submitting Online Complete the online form below and click the "Send" button at the bottom. Your request will automatically be sent to the Community Relations Department. Submitting by FAX Print and complete the Public Records Request Form. Then fax it to the St. Johns County School District's Community Relations Department at (904) 547-7523. Submitting by Mail Print and complete the Public Records Request Form. Then mail it to: Community Relations Department St. Johns County School District 40 Orange Street St. Augustine, FL 32084 Submitting In Person Print and complete the Public Records Request Form.
